Example using conditional logic

The following screen shots were taken from our built-in Right to Rent collection template. It begins with two form entry items, one asking for the respondent's personal details and the other asking whether they have any documents in their previous name.

After a couple of seconds the following new pack appears, advising the respondent towards their options for proving their name change. The Proof of legal name change pack has a condition that is only triggered when the response to the form above is 'Yes'.

Underneath the Claimed identity form, a new pack has appeared.

Depending on why their name changed, the respondent will have a range of documents they can use to evidence and excuse the discrepancy regarding their name. These documents must be inspected manually, which is a setting you can configure per document, and the respondent is advised accordingly.

The requestor's view

Here's what you'll see, as the rental agent in this scenario.

The requestor sees the response to the question posed as well as the pack for collecting evidence

Since the requestor will need to inspect and retain copies of any evidence, they are allowed to pick a file to upload using Pick file.

The other button adds a Not applicable option for both the requestor and respondent which lets them skip that item. This is not necessary in this case since the Proof of legal name change pack is set to accept any one of the items. This means, as soon as one file is uploaded and accepted, the pack is considered complete and will disappear from view.
